ASMBS State Chapters are able to apply for joint CME providership with the American Society for Metabolic and Bariatric Surgery. These documents will help State Chapters plan their educational activities in compliance with Accreditation Council for Continuing Medical Education (ACCME) standards.

The process of applying for joint providership begins 6 months before the start date of your CME activity. The timeline includes important dates, deadlines, and tasks to be completed in order to ensure your event meets all requirements for joint providership.

Joint Providership State Chapter CME Activity Agreement
This document, to be signed by both the State Chapter and ASMBS, establishes the responsibilities and terms of joint providership. You can download this document, complete it electronically, or print it out and scan/email to Kayla McCray.

CME Application and Activity Planning
This document is designed to ensure that CME activities considered for certification meet the Accreditation Council for Continuing Medical Education (ACCME) criteria.
Disclosure Report for Participant Notification
All faculty/speakers, planners, abstract reviewers, moderators, authors, co-authors and administrative staff participating in the continuing medical education programs sponsored by the ASMBS are expected to disclose to attendees any/all relevant financial relationships related to the content of their presentation(s) in accordance with ACCME Guidelines for Commercial Support. It is the Non-Accredited State Chapter’s responsibility to collect ALL disclosures and submit them to the ASMBS prior to planning.
ASMBS Financial Relationship Disclosure Form
If Application is provided at least 3 months in advance of meeting, ASMBS can assist in collecting financial disclosures for speakers. ASMBS must review all disclosures and review for conflict.
Letter of Agreement for Educational Grants and Sponsorship
This letter of agreement must be sent to all companies providing educational grants for the State Chapter event, stating the terms and conditions of any educational grants.
